Service delivery for this Department of Child Services (DCS) diagnostic and evaluative component will be comprised of a detailed demographics section including the child/adult’s name, chronological age, birthdate, educational level and names of schools, ethnicity, gender, and other pertinent referral information.
In conjunction with the demographics section, the biopsychosocial assessment (BPSA) will be utilized to gather specific information regarding the child/adult’s past and present life conditions.
In addition to the BPSA, a complete mental status exam will be incorporated into the clinical interview and assessment to further develop the information obtained from the BPSA. This information will assist in the formulation of a well-supported and appropriate diagnostic coding (if applicable).
The Clinical Interview and Assessment will include a statement regarding the youth/adult’s strengths and areas of personal growth, hopes and wishes, as well as their personal insights regarding helpful services and needed interventions. This statement will comprehensively summarize the presenting concern, current and past levels of functioning, interpretation of information obtained, treatment recommendations and service approaches, such as psychological evaluation, community support and interventions, case management, psycho-educational services, occupational rehabilitation or educational training, therapy/counseling, medication management, or advisement for a medical consult.
